1. Booking & Confirmation:

• Booking Requirements: All appointments must be booked in advance, either via phone, email, or our online booking platform.
• Confirmation: Once your booking is confirmed, you will receive a confirmation message. Please make sure your contact details are correct to avoid any confusion.
• Cancellations: We understand that life happens! If you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice. Cancellations made less than 24 hours before the scheduled appointment may result in a cancellation fee of 50% of the treatment cost.
• No Show Policy: If you fail to attend your appointment without notifying us, we reserve the right to charge the full treatment cost.

2. Treatment Information:

Glow Align offers a range of treatments, including facials, waxing, nails, and lashes and makeup. Our professionals are fully trained and use high-quality products to ensure the best results.

• Facials: Personalized skin care treatments to hydrate, rejuvenate, and improve your skin’s health.
• Waxing: We provide gentle waxing services for smooth, hair-free skin.
• Nails: From manicures to pedicures, our nail treatments are designed for relaxation and beauty.
• Lashes: Eyelash extensions to enhance your natural beauty.

If you have any allergies, sensitivities, or medical conditions, please inform our staff prior to your appointment to ensure your safety and comfort.

3. Late Arrivals:

If you arrive late for your appointment, we will do our best to accommodate you. However, please note that we may need to shorten your treatment time, or in some cases, reschedule depending on our availability.

4. Payment:

• Pricing: All prices are available on our website or upon request. Prices are subject to change, but we will notify you in advance.
• Payment Methods: We accept only card payments and EFT payments.
• Refunds: We do not offer refunds for services rendered. However, if you are unsatisfied with your treatment, please let us know, and we will work with you to resolve the issue.

5. Privacy & Confidentiality:

We respect your privacy. Any personal information provided during the booking process will be kept strictly confidential and will only be used for appointment-related communication.

6. Health & Safety:

Your safety and well-being are our top priority. If you are pregnant, have skin conditions, or any other medical concerns, please consult with your doctor before booking treatments. Our team will be happy to assist with any questions regarding the suitability of a treatment.

7. Photography & Social Media:

We love seeing your beautiful results! With your consent, we may take photos of your treatments for our promotional materials or social media pages. If you prefer not to have your photo taken, simply let us know beforehand.

By booking and receiving services at Glow Align Beauty Salon, you acknowledge and agree to the following:

1. General Acknowledgement:

• I understand that the treatments offered at Glow Align, including facials, waxing, nails, makeup and lashes, involve the use of products and equipment that are safe and professionally applied by trained staff.
• I confirm that I have provided accurate information regarding any allergies, medical conditions, or sensitivities that may affect my treatment.

2. Health & Safety:

• I understand that if I fail to disclose relevant medical conditions or allergies, I may be at risk for complications during or after my treatment.
• I acknowledge that certain treatments may cause temporary redness, irritation, or discomfort, and I accept that these are normal, minor side effects.

3. Liability Release:

• I release Glow Align and Style Hub and its staff from any liability for accidents, injuries, or adverse reactions related to the treatments provided, except in cases of gross negligence.
• I understand that Glow Align and its staff will take all reasonable precautions to ensure a safe and comfortable experience but cannot be held responsible for any complications arising from the treatment.

4. Consent for Treatment:

• I voluntarily consent to the treatments I have selected. I understand that I can ask questions or request clarification at any time before, during, or after the treatment.

5. Photos & Social Media:

• I agree to allow Glow Align to take and use photographs of my treatment for promotional purposes unless I specifically request otherwise.

6. Personal Responsibility:

• I accept full responsibility for informing Glow Align of any medical conditions, allergies, or sensitivities before my treatment. I acknowledge that I have the right to discontinue or modify any treatment at any point during the service.

7. Payment & Fees:

• I agree to the pricing of services as communicated and understand that payment will be due at the time of the treatment.

By signing this form, I confirm that I have read, understood, and agree to the terms and conditions outlined above. I release Glow Align Beauty Salon from any and all liability regarding the treatments provided.
